Please review this memo entirely and determine which scenario applies to you.
The Company submitted an employer filed claim report to the Kentucky Unemployment Insurance Division on behalf of all affected employees that were laid off during the previous week (Benefit Week 07/15/12 through 07/21/12). Because of the unexpected down week the list of affected employees was not able to be compiled and submitted to the state until the middle of last week. This in turn has caused a delay in receiving monetary determination notices and pay order forms in the mail from the unemployment insurance division. The monetary determination notice outlines your benefit amount, eligibility status, and effective date. The pay order form outlines when and how to request benefits. Once received please do not discard the monetary determination notice as you may need it for future reference. If you have not yet received these mailings from the state that does not mean that your claim has not been established. It is more than likely a mail delay. You will have until this coming Friday to process your benefit request (if eligible). The default pin number when using the telephone Voice Response Unit (1-877-369-5984) will be either the first or last four digits of your social security number. Anyone who previously established a pin number should attempt to utilize it before trying the default pin number option referenced above. You may also do your benefit request online, however if you do so please make certain that you remember to click on “Request Benefit Check”. Do not click on “File a Claim” as this will overwrite the parameters of your current claim. Instructions for online request are outlined on the backside of the unemployment division issued pay order form, the web address is uiclaims.des.ky.gov/ebenefit
Please be aware that under the new Kentucky unemployment insurance guidelines all new claims opened on or after 01/01/2012 will be subjected to a waiting week rule whereas the first week claimed within a given benefit year (which is a rolling 12 month period from the claim effective date) will not be eligible for a benefit check. However, you should request a benefit check even though it will not be issued. The reason being is that the waiting week rule requirement will have been met for affected employees, and any future claim weeks within the benefit year will be eligible for benefit payments provided all other eligibility requirements are met.
In order for a new claim to be valid affected employees must have previous quarter payroll earnings paid in Kentucky during the last 3 quarters of 2011 and the 1st quarter of 2012.This information will be disclosed on the monetary determination notice.
Any employee having a valid existing and/or extendable claim, which will be reactivated in this case, that was opened during 2011 with the Kentucky Unemployment Insurance Division (a benefit year is a rolling 12 month period) will not be subjected to the new 2012 waiting week rule referenced in the above paragraph. You will receive a pay order form which outlines when and how to request your benefit check. However, if you receive a new monetary determination notice in the mail that means that your previous benefit year has expired and your new claim will be subjected to the waiting week rule.
Those of you who have previously had a 07/01/2012 effective date unemployment claim opened on your behalf for the recent plant vacation shutdown period will not be receiving an additional monetary determination notice from the state for the most recent down week. If you were eligible for unemployment benefits per your monetary determination notice and you requested a benefit check for the first time for the plant vacation shutdown period then you have met the new waiting week requirement established by the state and will actually receive a benefit check based upon the information disclosed on your previously issued monetary determination notice provided all other eligibility requirements are met. You will receive a pay order form which outlines when and how to request your benefit check.
Employees that have worked 10 or more weeks since their most recent hire date for the Ford Motor Company - KTP will be filing against the Company's unemployment insurance account with the Kentucky Division of Unemployment Insurance. Those employees having worked less than 10 weeks for the
Company since their recent hire date will be filing through the unemployment insurance account of the Company, however these claims will be charged back to their previous employer’s account.
**Employees that began working at the plant prior to the end of the 1st calendar quarter of 2012 that had earnings paid to them from the Company (KTP) before the end of the 1st quarter, who most recently worked in a state other than Kentucky should be able to file a combined wage unemployment claim in Kentucky utilizing transferred previous quarter base wages from that state for the preceding quarters. This can only occur if you have payroll earnings reported to that state’s unemployment insurance division. Affected employees can contact me and I will assist them with these claims.
***Employees who began working at the plant after the beginning of the 2nd calendar quarter of 2012, who most recently worked in a state other than Kentucky will be required to contact that state's unemployment insurance division in order to utilize an existing or extendable claim (if applicable) or file for a new claim for unemployment benefits. Please contact the applicable state unemployment division for
claim application and benefit check / waiting week request deadlines.
Entry Level Employees are not eligible for Sub Pay benefits until they reach 1 year of service.

However, our contact person Chris Townsend at the Division of Unemployment Insurance - Shelby County Branch is willing to provide help to anyone that is eligible to file an unemployment claim in Kentucky. I will also be available to help any employee that is having difficulty with his/her unemployment claim and/or benefit check - waiting week request.
Business hours for the Shelby County Branch: Monday through Friday 8:00am until 4:30pm. Business hours at other branch offices may vary, so please contact your local branch for specific business hours.

Store on the right hand side of the roadway. After passing the Goodwill Thrift store, turn at the second
right (there is a Kentucky Government Sign at this location), then proceed through a small 4 way
