HOURLY OPTIONAL EMPLOYEE AND DEPENDENT LIFE INSURANCE
Limited Open Enrollment Period
March 9, 2011 thru April 25, 2011
As provided in the UAW/ Ford collective Bargaining agreement, active employees who currently have coverage under the "Employee and Dependent Optional Life Insurance Plan" may increase coverage by ONE level during the specified period
and
Employees with NO CURRENT COVERAGE may enroll for the 1st two levels of coverage, without providing proof of insurability.*
ALL EMPLOYEES CAN ALSO ENROLL IN THE OPTIONAL ACCIDENT PLAN AT THIS TIME!*
